- A valid passport (with a validity of 6 months)
- One passport-size photo
- Birth certificate
- 10th and 12th mark sheets
Nursing or Midwifery Diplomas and Registration Certificates
Verifications from every state or territory where the candidate practiced or was formerly registered.
If the applicant’s country uses state registration, they must have registered in each state where they practiced. The applicant must submit Registration certificates from each jurisdiction where they have practiced and/or been registered.
Work references attest to their moral character and good health. (Two employment references attesting to the applicant’s ability and character and attesting to their post-registration practice of at least 12 months.)
Transcripts of any applicable nursing or midwifery schools that the applicant has successfully completed. These must be correctly calculated in hours or must be submitted with a letter from the training facility that includes a conversion chart for credits to hours.
